Is the quoted price fixed?
Yes, it is stated before work starts. Surprises are discussed, they are not billed by default. Prices are in US dollars, invoiced by the LLC.
Who owns the code and the accounts?
You do. Code, repositories, accounts and domain names are in your name from delivery.
